Locker Check-out Form

               Band members are required to submit this form to be assigned an instrument room locker. All policies and procedures applied to instrument room privileges will be followed. A band member can lose his/her privileges to use this space. By submitting the form below, you agree to follow the guidelines of proper usage of the band instrument lockers. THIS IS REQUIRED.


Band Instrument Room Guidelines :

- Only COMBINATION locks are acceptable

- No loitering in the instrument room 

- Instrument case placed in the slot

- No sheet music in the locker

- No practice clothes, shoes, socks, or other garments placed in instrument slots without permission 

- The slot has a combination lock, and the combination is given to band directors

- No food items in lockers

- No trash in lockers under any circumstances

- Instrument cases remain in slots during rehearsals
