MHS BAND & GUARD  2019- 2020 TRAVEL PERMISSION & MEDICAL INFORMATION FORM 

               I (we), the parent/guardian give our permission for him/her to attend and participate in ALL SCHEDULED BAND & COLOR GUARD EVENTS & FIELD TRIPS FOR THE 2018-2019 SCHOOL YEAR. By granting our permission, we understand that all rules and regulations detailed in the MANOR HIGH SCHOOL BAND HANDBOOK will be in full effect during all trips, and support their enforcement by the MHS BAND STAFF. It is understood that violations of these rules may result in the suspension or termination of a student's involvement in field trip activities. Any violation of the Manor HS Student Code of Conduct may exclude students from trips. I (we) have read and understand the following District and Department Transportation Policies, and agree to adhere to these requirements: All students are expected to ride the bus to and from any field trip destination; In the event the student will need to use transportation other than the bus provided by Manor ISD, the Directors must have a written notice from the student's parent/legal guardian relieving the Directors and Manor ISD of any liability; Band and Color Guard students are required to ride the bus to any performance to ensure the timely arrival of all members for pre-performance preparation. When my student will need alternate transportation after a performance, I will contact the directors by a phone call or email including date; event; student name; WHO will drive the student (Students MAY NOT DRIVE THEMSELVES TO OR FROM ANY SCHOOL - RELATED ACTIVITY), and send it to the Director. This may be turned in at any time prior to the event or even at the event. Because these rules allow for accurate attendance to be checked, they will ensure that your student's whereabouts can be accounted for during any trips. 

 

Furthermore, I (we) grant any adult in possession of this agreement the right to seek and provide any medical attention deemed necessary in the event of a medical emergency.

Rules governing medications for students on Field Trips:

● All medication except asthma inhalers, insulin and epi-pens will be held by a responsible adult trip chaperone.

● All medication except asthma inhalers, insulin and epi-pens will be administered by a responsible adult trip chaperone.

● Students are permitted to carry their own over-the-counter medications.

● All school rules regarding medication administration apply during all trips.

● Parents may elect to have their students keep their over-the-counter medications in the hands of the chaperones. 

 

ADMINISTRATION OF OVER-THE-COUNTER MEDICATIONS

 

Students who expect the need for any non-prescription medications should bring those with them on a trip, either retaining those medications themselves or giving them to the chaperones for dispensing. On campus, Manor students may not carry or self -administer medications! On occasion, the unexpected need may arise for the administration of non-prescription medication per the permission of the parent(s) or guardian(s):
